No book about television production would be complete without a set of sample forms used to complete assignments and to stay on track with regards to production schedules, storyboarding and so forth. This page links to sample forms described throughout the book as well as to a test database containing sample questions and an answer key. Any teacher who has an interest in using these forms may download them for classroom use. So long as they are not used for commercial purposes.

You may also order a hard-copy workbook containing all the forms as well as all test questions with a complete answer key, by emailing me. Those who order the workbook will also receive, upon request, a disk in Microsoft Word format so that they can modify the order and content of the quizzes and accompanying answers to suit their needs.
